Policies

All patients are asked to bring a list of their medications or medication bottles to every visit. We suggest you make a list of your problems/questions and bring to your visit.

Copays and deductibles are due at the time of visit. You will be responsible for complete payment of services rendered at the time of visit without proof of insurance.
