Charges/Fees

There is fee for all paperwork requests that cannot be completed at the time of visit. The fee is $25 for up to 3 pages, $50 for up to 5 pages, $75 for more than 5 pages.There is $10 fee for all requests for work notes created outside of the time of visit.

 

All appointments require a 24 hr notice for cancellation. There will be a $40 fee for all no show appointments and a $75 fee for no show physicals. Your appointment will be considered a “no show” if you arrive more than 20 minutes after your scheduled appointment.
